Service Form

Please include this Service ID Form with every blanket order. This is one of the most important steps in preparing your blankets for cleaning.

As each blanket is processed, it is checked against your inventory list to ensure every item received is accounted for. It is not uncommon for customers to believe they sent a blanket for cleaning, only to later discover it was left at the barn, in a tack trunk, or stored elsewhere. A complete inventory provides an accurate record of the items submitted and helps prevent confusion about missing blankets.

For your protection and ours, we cannot accept responsibility for blankets reported as missing if an inventory list or completed ID form was not included with the order.
